Insureds not on Active Duty at the time of death:

 

If the insured was a Reservist or National Guard member, or had been released from Active Duty before his/her death, send a copy of the death certificate (or a photocopy) and a DD Form 214, Certificate of Release or Discharge from Active Duty with your claim.

Send a copy of the death certificate (or a photocopy) with your claim.


SGLI Disability Extension Claims

SGLI provides a free extension of coverage if the insured is totally disabled at the time of separation or release from active duty.

  • A total disability extension must be applied for directly to the OSGLI. To apply, simply complete SGLV 8715 - Application for SGLI Disability Extension.

  • For full-time members, if the insured is totally disabled at the time of separation, SGLI coverage is extended for as long as the total disability continues up to 2 years. At the end of the 2 year period or when the insured is able to return to work, the SGLI disability extension is automatically converted to VGLI coverage as long as the member pays the premiums.

  • For part-time members, if the insured incurs or aggravates a condition while on duty, coverage continues for 120 days following the period during which the disability was incurred or aggravated. The insured may apply for VGLI anytime during the 120 day extension.

VGLI Disability Claims

There is no provision for waiver of premiums or other disability benefits under the VGLI program.
